The things that actually scare me
most MDMA harm isn't about the molecule — it's about the circumstances. these are the biggest ones, and almost all of them are preventable.

Hyperthermia (Overheating)

MDMA impairs thermoregulation. Neurotoxicity is POTENTLY exacerbated by high body temperature. Hot environments dramatically increase risk.

how to keep it gentle

Stay in cool environments (20-22°C). Take regular breaks from dancing. Cool down with cold water on wrists/neck. Avoid crowded, poorly ventilated spaces.

Dehydration/Hyponatremia

Dehydration increases hyperthermia risk. BUT excessive water intake causes fatal hyponatremia (water intoxication) due to MDMA-induced ADH.

how to keep it gentle

Drink isotonic fluids (electrolytes), not plain water. 250-500ml per hour if dancing. Less if sedentary. Match fluid intake to sweat loss.

Adulterants/Unknown Substances

Street "MDMA" may contain dangerous adulterants (PMMA, cathinones, fentanyl) with completely different toxicity profiles. Purity is essential for any safety calculation.

Always use a drug checking/testing service if available. At minimum, use reagent test kits (Marquis, Mecke, Simon's). Fentanyl test strips recommended.

Pre-existing Cardiac Conditions

MDMA increases heart rate and blood pressure significantly. Pre-existing heart conditions, undiagnosed arrhythmias, or hypertension dramatically increase risk.

Individuals with known cardiac conditions should not use MDMA. If you have a family history of heart problems, consult a physician.

Polydrug Use

Combining MDMA with other substances dramatically alters the safety profile. Alcohol dehydrates. Stimulants compound cardiac strain. Some SSRIs cause serotonin syndrome.

Avoid all other substances during sessions. Be especially cautious of: MAOIs (dangerous), SSRIs/SNRIs (reduced effects, serotonin syndrome risk), stimulants, alcohol.

CYP2D6 Poor Metabolizers

Genetic variation in CYP2D6 enzyme affects MDMA metabolism. "Poor metabolizers" experience higher plasma concentrations and prolonged exposure, increasing toxicity risk.

Start with lower doses if unsure of metabolizer status. Consider pharmacogenomic testing. Be extra cautious if you've had unexpectedly strong reactions before.

Female Physiological Sensitivity
